Is this organization Tax Exempt under the regulations of Federal Income Tax Revenue service?
If YES send a copy of a determination letter, state exempt determination letter and a letter from IRS
Legal name of applicant, Print Name and Title, Date
I/We hereby certify that all the foregoing information is true and correct, and application for this grant is authorized by the Board of the undersigned applicant.
